Data Controller Application Form
In accordance with the Personal Data Protection Law No. 6698 (“Law”), data subjects transmit their requests related to exercising their rights “to learn whether their personal data is processed or not; to request information if their personal data has been processed; to learn the purpose of processing their personal data and whether they are used in accordance with their purpose; to know the third parties to whom their personal data is transferred in the country or abroad; to request the rectification of their personal data if it is incomplete or incorrectly processed and to request the notification of the operation carried out within this scope to third parties to whom their personal data has been transferred; to request the deletion or destruction of their personal data if the reasons requiring processing have ceased to exist, even though it has been processed in accordance with the provisions of the Law and other relevant laws, and to request the notification of the operation carried out within this scope to third parties to whom their personal data has been transferred; to object to the occurrence of a result against them by analyzing the processed data exclusively through automated systems; to demand the compensation of damages if they suffer damage due to the unlawful processing of their personal data” to Nilky İçecek ve Gıda San ve Tic. AŞ. in writing or by other methods determined by the Personal Data Protection Board.
To exercise your rights mentioned above, you can submit your request, which includes the necessary information to identify you and your explanations regarding the right you wish to exercise among those specified in Article 11 of the Law, by filling out the application form below, and delivering a signed copy of the form in person to “Esentepe Mah. For applications to be made to Nilky İçecek ve Gıda San ve Tic. AŞ. by third parties on behalf of the personal data owner, a copy of the power of attorney arranged for this matter, along with the completed Application Form below, and for applications made on behalf of children under parental authority/guardianship, a copy of the documents proving the parental authority/guardianship relationship, along with this Application Form, must be submitted.

FINALIZATION OF THE REQUEST Your application, submitted to Nilky İçecek ve Gıda San ve Tic. AŞ. by filling out and signing the above Application Form, will be finalized free of charge within the shortest possible time, and no later than thirty days from the date your request reaches us, depending on the nature of the request, in accordance with Article 13/2 of the Law. However, if the operation requires an additional cost, the fee specified in the tariff determined by the Personal Data Protection Board may be charged. Nilky İçecek ve Gıda San ve Tic. AŞ. will accept your request or reject it by explaining its reasons, and the answer will be notified to you in writing or electronically in accordance with Article 13/3 of the Law.
